Website privacy
Information on this page explains our website privacy policy and how we will use and protect any information about you that you give us when you visit this website.
This privacy statement only covers the Chelmsford City Health Primary Care Network website (https://www.chelmsfordcityhealthpcn.nhs.uk). It does not cover all sites that can be linked to from this site, so you should always be aware when you are moving to another site and read the privacy statement on that site.
What information do we collect?
From time to time, you will be asked to submit personal information about yourself (e.g. name and email address) in order to receive or use services on this website. Such services include bulletins, email updates and website feedback.
By entering your details in the fields requested, you enable Chelmsford City Health Primary Care Network and its service providers to provide you with the services you select. Any information you provide to Chelmsford City Health Primary Care Network will only be used by us, our agents and service providers and will not be disclosed unless we are obliged or permitted to by law to do so.
If you post or send offensive, inappropriate or objectionable content anywhere on https://www.chelmsfordcityhealthpcn.nhs.uk or otherwise engage in any disruptive behaviour on https://www.chelmsfordcityhealthpcn.nhs.uk, we may use whatever information is available to us, about you, to stop such behaviour. We will hold your personal information on our systems for as long as you use the service you have requested, and remove it in the event that the purpose has been met or when you no longer wish to continue your subscription.
The Data Protection Act
Under the Data Protection Act, we have a legal duty to protect any information we collect from you. We use leading technologies and encryption software to safeguard your data and keep strict security standards to prevent any unauthorised access to it.
We do not pass on your details to any third party or other government department.
Extended Access
As part of the 2022/23 contract with NHS England, GP Practices, as part of a Primary Care Network (PCN), have been asked to deliver extended access appointments (evening and weekend slots) to patients across the PCN.
These appointments will be provided by various practices within the PCN and will be open to all registered patients from the respective GP Practices within the PCN.
This means that, in order to deliver this service and provide the same level of care that you would receive from your own practice, we must share your GP records with those delivering the appointments to ensure they have access to all the clinical information about you that could be relevant to your issue. These clinicians will also record information about the consultation back into your registered GPs clinical record, ensuring that all of your primary care information is available in one place.
Partner PCN Practices that your information could be shared with are: Baddow Village, Beauchamp House, Stock & Whitley House Surgeries.
Your registered practice will remain data controller for this information, and all rights under the Data Protection Act and UK GDPR would be dealt with by them, as detailed elsewhere within this privacy notice.
Patients are able to decline this sharing, however this means that you cannot be provided with an appointment delivered by another PCN Practice in the extended timeslots, instead an appointment with your registered practice will need to be booked in normal hours.
Changes to our policy
If our privacy policy changes in any way, we will place an updated version on this page. Regularly reviewing the page ensures you are always aware of what information we collect, how we use it and under what circumstances, if any, we will share it with other parties.
Security
This site has security measures in place to protect the loss and alteration of information under our control.

Where services are delivered on the internet, this sometimes involves placing small amounts of information on your device, for example, computer or mobile phone.
